Tabitha Cephas is the Founder and Director of Tailored Care LLC in Dover, DE, where she has established a leading Personal Assistance Services Agency (PASA) services in the state of Delaware. She has over 20 years of experience as a Strategic RN Leader and Home Health Care Executive, Tabitha has built her reputation on delivering innovative patient care solutions and fostering high-performance teams. At Tailored Care LLC, she has forged strategic partnerships with major organizations, while cultivating and leading a dedicated team of over 80 healthcare professionals committed to providing high-quality patient-centered care.
In her previous roles she oversaw care coordination at Beebe Medical Center and Christiania Health System, where she implemented data-driven strategies that resulted in reduced readmissions and healthcare costs while optimizing patient care across a 906-bed Level 1 Trauma Center. As LTSS Supervisor at Highmark Health Option, Tabitha excelled in compliance, quality assurance, and strategic planning, earning recognition for her commitment to reducing health disparities and enhancing patient outcomes.
Tabitha holds a Bachelor of Science in Nursing from Wilmington University, and an Associate of Science in Nursing from Delaware Technical Community College. Her professional credentials are reinforced by her dedication to community service, as demonstrated by her founding of the Already Amazing Charitable Organization, which empowers individuals through essential goods, coaching, and education.
